Dear Parents,

At this time, we would like for you to please complete the 2020-2021 Opening of Schools Required Parent Forms.

All of the forms below are required and should be completed, signed and returned by Friday, September 25, 2020.

HOW TO RETURN THE FORMS

Option 1:   
Return all completed and signed forms as an attachment via email to our school registrar at soropeza@dadeschools.net.
**Include student name and ID number in the SUBJECT LINE of your email.

Option 2:
Return completed and signed documents in an envelope to the school Dropbox located in front of the main entrance of the building available from 8am – 4pm.

Option 3:
Return completed and signed forms via U.S. Mail to
